Patients are seen in our office Monday, Tuesday, Wednesday am, and Thursday pm.
All patients are seen by appointment and we request that
you call in advance so we may reserve a time for you. This
allows us to schedule adequate time with the physician for
participate in most insurance plans and will see patients
out-of-network if eligible. Please call the office for an
up-to-date list of participating insurance plans. All patients
are responsible for providing accurate insurance information
at the time of visit, including a copy of the most recent
Horizon Blue Cross
Horizon Omnia Tier 1

Workman's compensation and automobile carriers - several

The information received from the insurance company prior to your treatment is not a guarantee of payment. Please call your insurnace carrier if you have any specific questions about coverage.
If your insurance does not allow you to go directly to a
specialist, you must obtain a referral from your primary
care physician prior to receiving treatment. It is your
responsibility to obtain the referral before your visit
and to have it available with you at your appointment.
Payment for all co-pays, deductibles and self-pay balances
are due at the time of visit. Our office accepts only cash
or personal check with driver's license.
